Perineal Reconstruction in the Age of Robotic Surgery Thu, Oct 15 3:30-4:30 p.m. CDT

This panel performs a deep dive on decision making and reconstructive choices with a focus on flaps, and technical pearls and pitfalls for perineal and soft tissue pelvic reconstruction, including genital (vulva, vagina, penoscrotal) reconstruction, with emphasis on the evolution of donor site algorithms in the contemporary era where minimally-invasive ablative surgery has increasingly become standard of care

Upon completion of this learning activity, participants should be able to:

  1. Review common indications and anatomic units affected

  2. Review functional implications including sexual function

  3. Review reconstructive options with a deep dive into flap choices in settings of open vs closed surgery. Discussions of flaps will include advantages and disadvantages, what types of defects they can be used for, and challenges including reliability, ease of reach: Gracilis flap including various skin orientations, PAP flap, ALT flap, Pudendal thigh flap, Rectus abdominis

  4. Review management of common complications: Wound dehiscence and extended healing, Pelvic fluid collections, Chronic peritoneal perineal fistulae, Flap loss

  5. Review particular challenges of the obese patient, total vaginal reconstruction, radical defects
