Upper third facial feminization plays a central role in aligning facial features with gender identity and patient expectations. Variations in frontal sinus anatomy, brow position, hairline characteristics, and soft tissue envelope present technical challenges that require careful preoperative planning and precise execution. This session will review contemporary surgical techniques for forehead contouring, brow reshaping, and hairline advancement, with emphasis on anatomic considerations, fixation strategies, and intraoperative decision making. Evidence informed discussion will focus on complication recognition, prevention, and management to support safe, predictable, and patient-centered outcomes in adult gender-affirming surgical practice.

Upon completion of this learning activity, participants should be able to:

  1. Describe the relevant upper third craniofacial anatomy and anatomic variations that influence surgical planning in facial feminization procedures.

  2. Compare operative techniques for forehead contouring, brow repositioning, and hairline advancement, including indications and technical considerations.

  3. Analyze risk factors associated with common complications such as contour irregularities, frontal sinus violation, alopecia, and sensory changes.

  4. Develop a comprehensive operative and postoperative management strategy to minimize complications and optimize aesthetic and functional outcomes.
