Extended L-Framework: An Innovative Technique for Reconstruction of Low Nasal Dorsum by Autogenous Costal Cartilage Graft

I have an incredible passion regarding the topic of rhinoplasty. While working in this field, I have been confronted by many patients complaining of low nasal dorsum due to either developmental or acquired etiologies, I have decided to do something to relieve the suffering of those patients. In reviewing the literature, dorsal nasal augmentation utilizing either prosthetic implants or autogenous tissues, are the most popular techniques that have been used. I did some innovation in autogenous reconstruction of the low nasal dorsum in the form of Extended L-Framework using Costal Cartilage Graft to improve the outcomes regarding the natural appearance, long-lasting results and to avoid the "operated look." This technique has the advantages of being more anatomical and preserves the pliability of the lobule with consistent, reproducible results.

Nipple Reconstruction: A Novel Triple Flap Design

The idea for this triple flap design for nipple reconstruction came from a patient case, where the current surgical techniques would have given a suboptimal result. Our first patient had lost her nipple due to necrosis after duct excision, however, areola remained with normal shape and size. Other techniques would have changed the shape of the areola from round to oval, but the triple flap technique recruits tissue in a more circular fashion and provides a broader base to the flaps. When the design was developed we found the technique promising, with a potential of improvement of the outcomes of nipple reconstruction. With this background we conducted the present study to evaluate the feasibility and early postoperative results with regard to complications and nipple projection as the first step in the evaluation of this novel technique.

Sigma-lead Male-to-Female Gender Affirmation Surgery: Blending Cosmesis with Functionality

Current male-to-female(MtF) sex-reassignment-surgery techniques have not been fully successful to achieve the ideal objectives. Patients report persistence of genital dysphoria with severe problems in terms of vulva aesthetics and functionality of the vagina during sexual intercourse leading to high rate of dissatisfaction and rejection by the partners. The "help-letters" from patients who had been operated with pre-existing techniques prove this point. Our innovative “true shape sigma-lead SRS: Kaushik’s technique,” has overcome the functional as well as aesthetic inadequacies for outer genitalia and vagina and has now become the technique of choice for MtF genital SRS for our patients. It has proven to be safe and reliable technique and allows faster healing, minimal dilation, and nearly natural vulva and vagina with lubricating and orgasmic capabilities. Patients who had sigma lead primary or corrective SRS have reported alleviation of genital dysphoria with extreme satisfaction and happiness in the form of successful relationship and married life. This is perhaps the largest reported series of rectosigmoid use in transsexuals carried out for primary vaginoplasty.

We aimed to develop a better technique to estimate resection volumes that would benefit plastic surgery trainees and junior surgeons in performing such a procedure more accurately, and also aid in satisfying patient curiosity during the initial consultation with respect to how much tissue might be excised. Our results can assist plastic surgeons in predicting the amount of breast tissue to excise to achieve a given cup size. A secondary use of these results is a breast volume guide for implant planning.

A Simple and Low-cost Method of Sleeve to Insert Silicone Gel Breast Implants

Breast augmentation with silicone gel implants is one of the most common breast surgery procedures. It is well described that the contact between skin and implants is described as an agent of and subclinical infection, and biofilms have also been associated with complications. In order to avoid contact between implant and skin/parenchyma, some authors introduced the concept of an implant conduit made of some materials. Others authors in the 90’s, described the "no-touch technique", based on retractors to hold the incision open and avoid skin contact. More recently many plastic surgeons consequently advocate a no-touch technique in order to minimize skin/parenchyma contamination. This is generally accomplished with a device composed of a nylon sleeve with a hydrophilic inner coating. Thus, and based on this concepts, this present article describes a low-cost sleeve adapted to insert smooth, nano-textured, and texturized implants. Our study propose an easily available option: a sterile plastic saline container that is elastic, transparent, and protects the implant during insertion. These containers are available in 500-1000 cc sizes, and is a non-PVC bag composed of several polyolefin layers. Thus our impression is that this new sleeve allows the surgeon to perform breast augmentation/revision using the no-touch technique, in a simple, reproducible, and in a more economical manner.

Total Breast Reconstruction with Fat Grafting Combined with Internal Tissue Expansion

The main inspiration to write this paper come from my observation of disadvantages of breast reconstructive methods used so far. I also found some researches suggested that remote ischemic preconditioning recipient tissues improves the viability of fat transfer. This gave the idea to apply internal expansion before the fat grafting. I am aware that this is not the final solution to the problem of breast reconstruction but it shows the direction in which regenerative surgery can go.

Cultural Competence and Ethnic Diversity in Healthcare

I was inspired to write this article based on my experience and challenges as I reflected on my journey as a black female immigrant in academic plastic surgery. I observed the paucity of diversity in the field and in healthcare. This firsthand experience motivates me to continue to mentor and sponsor individuals and trainees across all racial, ethnic, and gender backgrounds to positively impact our next generation of healthcare providers. In this paper, I work with one of my mentees to crystallize these discussions regarding the current problems in these areas and proposing practical solutions. Developing and supporting individuals and trainees in the areas of cultural competence and ethnic diversity helps to better serve a diverse patient population, improve patient outcomes, and create an environment of success for trainees, physicians, and patients across all gender, racial, and ethnic backgrounds.

Most surgeons are reluctant to apply true vertical mastopexy with full wedge excision to simultaneous augmentation. Doing so risks a higher capsular contracture rate due to cut parenchyma placed against the implant and even implant exposure. I found after years of experience that Mladick's muslce splitting technique solves the dilemma by completely isolating the implant with a vascular layer from the cut parenchyma above.
